If you believe that CELS is the most appropriate READ assessment for a student in K-3, but they do not meet the eligibility criteria, please complete this form to have the student manually added to the CELS eligibility list. Please note that CELS is designed only for students with significant cognitive disabilities.  This form must be completed by the principal or the SPED case manager for that student.
