Alex Aust: Primary Coach Action Assessment
INSTRUCTIONS:

Now that the simulation is over, it's time to capture the most important feedback for your assigned participant.  Be prepared to review this form online with your participant.

1. Review your notes and find areas where the participant was most effective, and where he/she could have been more effective.
2. Complete the ratings portion based on your WHOLE DAY'S observation.
3. Identify the top 3 strengths you observed, supported by specific feedback.
4. Identify the top 3 areas for improvement you observed, supported by specific feedback.
5. Create a short summary that can include any coaching thoughts, suggestions, and words of encouragement.

Once you complete your feedback, hit the SUBMIT BUTTON.  You will see the option to review your responses. You can use this to walk through the coaching session with your participant.

THE ACTUAL COACHING SESSION:

Open the conversation by introducing yourself, and ask:

•How do you think it went for you?
•What did you see as your successful moments?
•What did you struggle with the most?

Move into the assessment reviewing your feedback and thoughts, giving your participant a chance to react. Ask questions like:

•What stands out to you about the feedback?
•What's the good news?
•What realizations do you have?
•What's surprising?

When it's time to summarize, give the participant a chance to download any thoughts, frustrations, etc.  Encourage them to process the experience as you offer recommendations and encouragement. End with a thank you.

Note:  Participants will get an email copy of their assessment.
