Book Arts Jam 2019 Exhibitor Registration
To apply:
1) Print, review and sign the Terms & Conditions (T&C) form
2) Fill out and submit the form below.
3) Mail the signed T&C with your check ($85 payable to "Bay Area Book Artists") to the address in the T&C.

Because space is limited, applications will be considered based on date of both the submission of this form, and also the receipt of the signed T&C and check. Checks will be returned to any Exhibitor who does not get a table.

Exhibitor tables will be 8 feet long.
