Program Change Form
 Due to mandates it is important that class sizes be maintained. We ask for your help and participation in keeping to the schedule you have been assigned. Schedule changes will only be approved for the following reasons:

1. The student has attended summer school and now qualifies for a different class.

2. A required course is missing from the schedule (i.e. English)

3. Currently program for a course that has previously been taken and passed

* If one of the above reasons apply to you, please complete the form below

Appointments will ONLY BE MADE by the counselor, if the schedule change proves to be more complicated. The Counseling Offices will not be available for drop in appointments. Counselors will email students once the change has been made. Please submit your Program change forms NO LATER than September 13, 2024.

* Once the Program change form is received by the counselor, the counselor will have to confer with the Director or Coordinator of that subject area for approval.
