How it works: Complete the following electronic Health ScoreCard and associated fields. Submit supporting documentation or materials via email to firstname.lastname@example.orgOnce your Health ScoreCard form and supporting documentation have been submitted, your Community Health Excellence Award submission is complete.
Within 5-days of submission, MiHIA will confirm if your submission has been received. The MiHIA Evaluation Team will review and calculate scores following the 2018 submission deadline; additional information may be requested at that time. MiHIA will also offer guidance and support as you and your organization set an organizational 3-year improvement goal and develop an action strategy.
Submissions for 2018 must be received by September 1, 2018.
