If you are already an affiliate staff member and wish to add to or change your current teaching responsibilities, please complete this form.

For example, if you need to add or change the modules or programmes you are teaching on, teach for another faculty or wish to be approved as a Programme Leader.

Your application will then be put forward for approval by Faculty Quality Teams. Please ensure you complete this form by following the given instructions and providing correct information with assistance of your HE Manager. Incomplete information will delay the approval process.
