Electronic Signature Needed: As a condition of participating as a vendor, exhibitor, band member, participant or volunteer at the Soulsational Music & Wellness Festival (the “Festival”), I, acknowledge that I fully understand the risks inherent to participation in the Festival, and I expressly agree, on behalf of myself, my heirs, executors, administrators, successors, and/or assigns, to release and hold harmless Soulsational Music & Wellness Festival, Venue location, Alternative Health Solutions of NJ, Michelle Leonard and their respective subsidiaries, affiliates, agents, officers, directors, volunteers, managers, other vendors, and employees (each of the foregoing, collectively, the “Releases”) from and against, any and all damages, losses, expenses (including attorneys’ fees and costs) or injuries, and any and all other claims of any type whatsoever for any personal injury, death, disability, or damage to tangible or intangible property, or any other loss, damage, or injury of any nature whatsoever (collectively, “Claims”) relating to or sustained or caused during the Festival. Furthermore, I agree, on behalf of myself, my heirs, executors, administrators, successors and/or assigns, to indemnify and hold harmless the Releases from and against any and all Claims made by any third party arising out of my participation in the Festival. Such obligation shall not be construed to negate, abridge, or reduce other rights or obligations of indemnity which would otherwise exist as to any party or person described in this Release and Indemnity Agreement. I agree to keep confidential and not to disclose any non-public information, regarding the festivals or any of the Releases, disclosed to me during the course of my participation in the Festival. I give and grant to Soulsational Music & Wellness Festival, Alternative Health Solutions of NJ, Michelle Leonard and its officers, parent companies, employees, agents, representatives, affiliates, subsidiaries, consultants, independent contractors, successors, assigns, designees and licensees ("LIG"), the right to use and publish my name, nickname, voice, picture, portrait, likeness, characteristics, live or recorded performance, photograph, signature or facsimile thereof, and biographical materials (the “Identity”) as embodied in materials associated with Soulsational marketing, advertising, sales or promotional efforts (the “Materials”) in all forms of media now known or hereafter created, including without limitation, video, television, print, billboards, point-of-purchase materials, corporate communications, internal and external marketing pieces, and Internet in any and all territories throughout the world. In the event that any clause, term, or provision of this agreement shall be deemed or adjudicated void or invalid, it shall in no manner affect the validity or enforceability of the remaining clauses, terms, and provisions herein which shall remain in full force and effect. I hereby acknowledge that I have read the Release and Indemnity Agreement and am satisfied that it is fair and equitable. I shall not revoke this consent and approval. Electronic Signature Needed. Registrations are honored on a first come, first served basis. Out of respect for all those supporting the festival, spaces will not be held, and all items listed below are required before a registration is complete. If payment is not in full one week before Festival day you will not be permitted to set your space up. If your payment plan is not maintained in a timely manner your fee will increase to full rate fee of $250. Soulsational festival does not grant exclusivity. Soulsational reserves the right to limit and separate vendor spaces for repeating services or products. This is my Electronic Signature.

Electronic Signature Needed. Vendors must bring their own supplies for set up. Tables, tents, electrical cords, and all items you bring should be labeled for easy identification. We do have the option for you to rent the table and chairs for the day. We ask all exhibitors to remain completely operational through One before last stage act is scheduled to finish. We welcome you to stay open till the end of the festival. You are most welcome to stay later if you would like, the festival continues with music until time as listed above. In order to give our exhibitors the greatest exposure for their business, we are limiting reservations to three practitioners and or business in any one field per county, and only one vendor per product per county. Examples are Mona Vie, Arbonne, and Scentsy. No other exclusivity will be offered. We ask all those attending to treat the festival as their personal event and advertise and market their involvement. As we strive to create a festival of good will and integrity, we reserve the right to deny registrations at our discretion if they conflict with the overall vision of the festival. Please note this is a rain or shine event. No refund for any reason will be offered or granted. You understand All Soulsational Vendors must be register with the State of New Jersey Tax Department and have their Tax Certificate with Tax ID Number on display at the Soulsational Festival (website on top of this form) and you may want to include festival insurance. I have read the entire Liability Release above. Exhibitors and its volunteers, employees and subcontractors shall indemnify, defend and hold harmless Soulsational Music & Wellness Festival, Veterans Park, Berkeley Township, Alternative Health Solutions of NJ, Michelle Leonard, and all volunteers and subcontractors from and against claims, litigations and charges for any personal injury or property damage resulting from or arising out of Soulsational Music & Wellness Festival. Fees actually go to the festival prodcution. This is a free to attend event with no ticket sales. "Produced by the community and for the community." Earlier registration is better. Prices do rise as we get closer to the festival date. We are a volunteer team of coorinators.
