This form is to request state assessment scores, report cards, and/or middle school transcripts for students who are no longer enrolled in Cupertino Union School District. Middle school transcripts should only be requested under special circumstances, such as if required for a program or a new school. If you need a certified transcript, please contact the middle school.

Please allow 7 business days for your request to be processed. The requested data will be sent only to the parent email address on file for privacy and security reasons.

If your student is currently enrolled in a California public school, please check with your school district for access to CAASPP scores.
