To celebrate we are hosting a once in a lifetime special event to say THANK YOU. On 11-2-14 we will randomly select participants from this survey for a FREE 15 minute professional in-studio photo shoot.

Have the complete experience with a studio portrait session in NYC! Want to look your best, a professional make-up artist will be on site for a low cost. Want to feel relax for your photo shoot, professional massage therapist will be on-site for a chair massage.

This survey will be utilized to randomly select participants for the FREE Holiday Session in-Studio Photo Shoot hosted by Omar Lopez Photography Studios on 11-2-14 at the studio on 125th & Park Ave in NYC. .

Data collected here will be shared with our participating sponsors. Must be 18 or order to participate and a resident of the State of NY, NJ or CT.
