Dayle's Custom Wrap Order Form

The cost of each sheet is $22.00 (based on a projection of 8+ sheets ordered). This includes the tax & shipping cost (to me) from Jamberry. For each address where the wraps are to be sent, there's an additional $2.00 fee to cover my cost to ship to you. Money can be sent via PayPal to daylelynne@gmail.com (please select "friends and family"). Using PayPal allows me to offer you the best possible prices!

***If you want to wait to send payment until all of the orders are in and I know I'll be placing the order right away, that's fine! Just leave me a message in the Special Notes section (with your preferred contact method), and I'll contact you when I'm ready to place the order. I only ask that you only fill this out if you are committed to making the purchase so that it doesn't throw off the order for other customers!***

If you would prefer to pay via gift certificate, they can be ordered here - http://daylelynne.jamberrynails.net . . . . Gift certificates are only available in $25, $40, $60, and $100. Unfortunately, I cannot use part of a gift certificate when placing the order, so if you do choose to pay this way, I'll throw in some extra swag! (Please put the gift certificate number in Special Notes section.)

I *will* accept gift certificates from other consultants for $25/sheet and a minimum of 2 sheets ordered. This includes my cost to ship to you.

Once I receive order for 8+ sheets, I will place the order and let you know via email.
