2025 Health Empowerment Celebration Volunteer Sign-Up Form
RaisingHealth (formerly AMPHS) is excited to bring our annual Health Empowerment Celebration to Sunset Park to encourage community members to take control of their own health and wellness. There will be activities, performances, resources, and more for the community to enjoy!

We are urgently seeking at least 50 volunteers like you to make our event a success! Please register below to select a shift. If you have additional resources (donations, transportation for setup, etc.) or specialized skills (health background, safety background, etc.) you would want to contribute, feel free to elaborate in the response of the last question.
