High Ability Identification Review Request
Please complete and submit this form with all information requested if you would like for your child to be re-evaluated for high ability identification in our school district. Submit only one form per child.

Please complete this form and submit all additional documentation/evidence via email to your child's principal by March 24 at 9:00 AM, to initiate an appeal in the current school yearAppeals submitted after the above date/time will be reviewed beginning in September of the next school year.

The review request must be accompanied by documentation and/or samples of student work that demonstrate the student’s achievement and/or aptitude.  More information about additional documentation/evidence is given later in this form. 

It is essential that review requests and documentation/evidence are submitted by the above date so that WLCSC staff may complete the review in time to gather information for class placements. Review committee decisions are final.
