Thank you for your interest in the School to Work Alliance Program (SWAP). SWAP is a choice service and this means the youth has to choose to engage in services.  This application is meant to be completed by the YOUTH who is interested in applying for SWAP services; not the parent, guardian, or educator. Questions in this form will help determine if you may be eligible for SWAP Services. Once we receive your application the SWAP Specialist assigned to your area will contact you to discuss next steps.
