Diploma & Cap & Gown Order Form
It is time for the academic office to order diplomas.  We need to have your student's FORMAL GIVEN NAME as it is to appear on his/her diploma.  Please use upper and lower case and include any punctuation.  This is the name that will be read at the graduation ceremony.  This should include "Jr.", "III", commas, etc.
