Code of Conduct & Waiver Release Form

This Code of Conduct and Waiver Release Form aims to ensure a positive, fair, and respectful environment for all participants in ForgeHub Tournaments. Thank you for your cooperation and commitment to upholding these standards.

Please read through the following Code of Conduct carefully. By checking each box, you acknowledge that your team entire team has read, understood, and agree to abide by the terms outlined. *

1. Respect and Integrity: Treat all participants, staff, and spectators with respect and courtesy. Refrain from using offensive, discriminatory, or harassing language and behavior. Acknowledge and respect the diversity of our community, including differences in gender, race, ethnicity, sexual orientation, and ability.

2. Fair Play: Compete honestly and honorably, without cheating, exploiting, or using unauthorized software or hardware modifications. Respect the decisions of referees, officials, and organizers, and follow the rules and guidelines set forth for each event.

3. Participants of the tournament will be held to a high standard of behavior, communication, and action. Participants are expected to represent the tournament in a professional and courteous manner and are strictly prohibited from engaging in communication or actions that could be considered vulgar, toxic, antagonistic, inflammatory, threatening or generally misrepresent the Administration in relation to the Tournament at any time. Examples of prohibited behavior: Hate speech or discriminatory behavior, threats, sexual harassment of any kind, racist or sexist words or phrases, extreme profanity, any "sound-alike" or "look-alike" words or phrases that reference these topics, any other type of conduct deemed inappropriate at the discretion of the Administration or tournament organizer

1. Media and Likeness Release: I grant permission for LVTHalo and ForgeHub to use my photos, videos, and likeness for promotional purposes on their social media channels and other media content. I waive any rights to royalties or other compensation arising from or related to the use of my image or recordings.

2. Accuracy of Information: I affirm that all the information I have provided is accurate and truthful. I acknowledge that providing false information can result in my disqualification.

3. GamerTag Compliance: I agree to compete using the GamerTag provided on this form. I acknowledge that using a different GamerTag will result in forfeiting my spot on the team and possible disqualification.
