River Trails School District 26 has partnered with the University of Illinois to test School District students, teachers, and staff members for COVID-19 infection. Due to vaccination status and mitigations in place, many parents chose to opt their children out of testing. Due to recent mitigation policy adjustments, parents now have an opportunity to opt children back in for testing by completing this form. Information will be adjusted in the Shield database to allow the child to take part in weekly testing. Please allow a few days for processing prior to testing being reinitiated.
