This form is to be used to formally report all incidents having occurred within The Water Well Project or one of its supported Projects, either confirmed or alleged.

This includes:

Workplace Health, Safety and Wellbeing incidents. For guidance, please refer to The Water Well Project Health, Safety & Wellbeing policy, the CovidSafe volunteer handout and the Working Remotely policy.
Complaints, dispute, grievance made about us, our services, staff or volunteer, a work related problem. For guidance, please refer to The Water Well Project Complaint & dispute policy.
Unacceptable workplace conduct including discrimination, bullying, harassment, victimisation or gossip. For guidance, please refer to The Water Well Project Workplace discrimination & harassment policy.
Abuse, neglect or exploitation of vulnerable people. For guidance, please refer to The Water Well Project Safeguarding policy.
Incident involving children’s protection. For guidance, please refer to The Water Well Project Child Safety policy.

Every report will be investigated and you will be contacted if you have provided your details.

If a person believes that another person is at risk of immediate harm or the victim of a criminal offence, they must immediately contact 000.

Confidentiality

At The Water Well Project, confidentiality is a key principle of reporting and managing concerns and incidents.

The names of people involved in a case will remain confidential knowledge held only by the person investigating the incident. This identifying information will be divulged only on a “need to know” basis, or when required by Australian law, or when a report to police or child protection authorities is made.
