Adult Summer Reading Club Form 2025
Please make sure you have registered for the Adult Summer Reading Club before filling out this form. Contact the library to register at 631-928-1212. Then use this form to submit your book reviews.  All 3 reviews must be submitted at one time.  Questions? Email us at aref@cplib.org .

Forms must be submitted by Thursday, August 21st. All those who complete and submit the form will receive 3 raffle tickets and a bingo card. Complete 5 out of 10 activities on the bingo card and receive 5 more raffle tickets. Raffle will be held on Friday, August 22nd. You do not need to be present to win.
