Central Avenue LED Street Lighting Survey Questionnaire
The Lighting Research Center (LRC) at Rensselaer Polytechnic Institute, in partnership with the Town and Village of Colonie, the New York State Department of Transportation, the New York State Energy Research and Development Authority, National Grid, the Capital District Transportation Committee, the University Transportation Research Center, and Creighton Manning, is evaluating new LED street lighting along Central Avenue approximately between Madison Avenue (near Fuller Road) and Reber Street (near Midway Fire Department). We would like to know your opinions of the new street lighting.

The lighting along Central Avenue has been divided into five sections:

• Between Reber Street and the entrance to Colonie Plaza (includes Golden Corral, Colonie Animal Hospital)
• Between the entrance to Colonie Plaza and Vly Road (includes New Karner Road, CarMax, Colonie Diner)
• Between Vly Road and Lincoln Avenue (includes Pep Boys, Otto Cadillac, ShopRite, Colonie Fire Company, Krause's)
• Between Lincoln Avenue and Wolf Road (includes Verizon, Albany Mattress, Cole's Collision, I-87 interchange)
• Between Wolf Road and Madison Avenue (includes Lowe's, Target, Goodwill, Fuller Road, Kurver Kreme)

You can answer the survey for any and all of the sections you have visited since the LED streetlights have been installed. LED streetlights produce white light, while the previous sodium streetlights produce yellow/orange light.

Completing this survey is completely voluntary. Please observe the street and the lighting at night, and then for each section of Central Avenue you are familiar with, select the responses which most closely describes how much you agree or disagree with each statement.
