Family Story Submission Form
Thank you so much for considering sharing your family's congenital lung malformation experience. Family stories help other parents facing a new lung malformation diagnosis and they may also be a resource for medical professionals who want to learn how best to support families in this situation. Please read this form carefully before submitting.

What happens after I submit my story?
We’ll be in touch shortly after you submit your story to let you know next steps. Our goal is to share a wide range of family experiences with CPAM, BPS, and other lung malformations. We will edit your story for clarity and length and so that it fits the ethos of CPAM Parents: medically clear and accurate while honoring each family's experience. We will always share your edited story with you before we make it public to see if there are any changes you would like to make. We’ll also ask if there are any photos or other items (such as a table of CVR numbers from appointments) that you’d like us to include. 

Do I have to use real names?
When you submit your family’s story, we ask for real names. This helps us build trust, ensure accuracy, and makes it possible to follow up with you if we have any questions about your submission. For anything we share publicly, we will always confirm with you how you’d like us to refer to you and your child. You can choose to use real first names, initials only, or remain fully anonymous by using pseudonyms.

Consent to Share Your Story
By submitting your family's story, you give CPAM Parents permission to publish it on our website and excerpts in related materials. You understand that this information will be publicly available. We may edit for clarity or length, but we will not change the meaning of your story. Please avoid including any details you do not want shared publicly. After publication, you can request for your story to be edited or deleted at any time.
