In consideration of being permitted to participate in the Community Outdoor Dance Class at Celebrate Mantua, I, the undersigned, on behalf of myself, my heirs, personal representatives, and assigns, hereby agree to the following:

Assumption of Risk I acknowledge and understand that participating in the Event involves physical activity and could result in physical injury. I voluntarily agree to assume and and all risks associated with my participation, including but not limited to, falls, injuries, contact with other participants, and the effects of weather, traffic, and conditions of the event area.

Waiver and Release I hereby release, waive, discharge, and covenant not to sue DANCE IQUAIL! Inc., The Mantua Enterprise for Community Culture and Artistry, D. Richardson Productions, the City of Philadelphia, and their respective officers, directors, employees, agents, affiliates, donors, partners, representatives and volunteers ("Released Parties") from any and all claims, liabilities, demands, damages, costs, expenses, actions, and causes of action of any kind or nature, known or unknown, arising out of or related to any injury, loss, or damage that I may sustain as a result of participating in the Event, including but not limited to any injury or loss that may be caused by the negligence of the Released Parties.

Indemnification I agree to indemnify and hold harmless the Released Parties from any and all claims, demands, losses, liabilities, damages, and costs, including attorney's fees, arising from or in connection with my participation in the Event.

Medical Treatment I consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during the Event. I release the Released Parties from any claims or liabilities arising out of or related to any first aid, treatment, or medical service rendered in connection with my participation in the Event.

Governing Law This Release and Waiver of Liability shall be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania.

Severability If any portion of this Release and Waiver of Liability is found to be invalid or unenforceable, the remaining portions shall continue to be valid and enforceable.

I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

IMAGE RELEASE  CELEBRATE MANTUA

I hereby grant permission to DANCE IQUAIL! And their respective representatives and project partners, the unlimited, perpetual and irrevocable right to take or capture (including by photography, videotape, film or any other medium), and use my image, likeness, name and/or speech in connection with DANCE IQUAIL!’s activities and events being produced in or for  Philadelphia, Pennsylvania, without any compensation or any other consideration. This project is known as CELEBRATE MANTUA. 


I hereby grant permission to DANCE IQUAIL! And their respective representatives and project partners to create and use any and all reproductions of this project in any manner or media and for any purpose they see fit, including in any and all of their publications, including websites. I understand that all such materials will be the property of DANCE IQUAIL!.


I hereby authorize DANCE IQUAIL and their respective representatives and project partners to edit, alter or otherwise modify the project as it relates to my involvement in Celebrate Mantua and grant them the right to use or not to use my image likeness and or speech in connection with the related publications and/or reproductions. In addition, I waive the right to inspect or approve all such reproductions. I hereby waive any right to royalties or any other compensation arising from or related to the foregoing uses. 


I hereby release and hold DANCE IQUAIL and any of their respective directors, officer, agents, employees and customers and appointed advertising agencies, harmless from all claims of any kind that, I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on the behalf of my estate might otherwise have in connection with any of the forgoing uses of my image, likeness, name and or speech. 


I have read the foregoing and fully understand the contents thereof and I freely agree to the terms of this agreement.
