Bethune Blaze Service Request Form

Bethune Blaze is a club designed to prepare students to assist in audio and lighting duties during assemblies and special events. We can not guarantee all professional equipment and/or service during service requests. If Bethune Blaze is unable to meet any specific requests, we will make alternate arrangements and/or notify you before the event.

Please ensure that this event was approved by the appropriate supervisors BEFORE filling out this form. We may be in contact with the staff advisor and administration to ensure that all approval for the requested event has been previously obtained.

This form must be completed at least seven (7) days prior to the event. If it is not filed seven (7) days prior to the event, this form cannot be used. A paper form must be obtained from the main office and written consent must be obtained from the Bethune Blaze Presidents.

If any corrections to this request are needed to be made, please notify a Bethune Blaze executive or email blaze.bethune@gmail.com as soon as possible. Changes that are made with less than seven (7) days prior to the event may not be fulfilled. Please note that this request is subject to approval by Bethune Blaze and not all requests made are guaranteed to be fulfilled.

Note that rehearsals and events are treated as SEPARATE events and multiple form submissions are required.
