Consent to Participate

I, the undersigned, confirm that I am the parent/legal guardian of the above-named child. I give consent for my child to participate in functional strength and conditioning activities that are age-appropriate and designed for physical development. I understand that while these activities are planned and supervised with safety as a priority, all physical activity carries a risk of injury.

I confirm that my child is physically fit and well to participate and that I will inform the organisers immediately of any changes to their health or fitness status. I understand that it is my responsibility to consult with a medical professional if I am unsure about my child’s ability to participate in physical exercise.

PAR-Q (Physical Activity Readiness Questionnaire)

I agree to complete a PAR-Q form accurately before my child begins any training, and to update this if any medical circumstances change. I understand that this form is a screening tool and not a substitute for medical advice.

Medical Consent and First Aid Treatment

In the event of an accident or emergency, I give permission for qualified staff to administer first aid and/or seek appropriate medical treatment for my child. If I am not present, I authorise the organisers to act in my place and agree that they may make decisions on my behalf to ensure the safety and wellbeing of my child. Every effort will be made to contact me at the earliest opportunity.

I understand that while all reasonable care will be taken, the organisers, coaches, and staff will not be held liable for any injury or illness unless caused by proven negligence.

Safeguarding Statement

The safety, wellbeing, and dignity of all participants is a priority. Staff and coaches working with children are vetted and trained in safeguarding, and activities are delivered in line with relevant UK legislation and best practice guidance. Safeguarding concerns will be handled in accordance with our safeguarding policy and reported to the relevant authorities where necessary.

All personal information collected will be stored securely and used only for the purpose of administering sessions, safeguarding participants, and meeting legal obligations. Information will not be shared with third parties without explicit consent unless required by law. You have the right to access, correct, or request deletion of your data at any time. Media Consent *

Photographs and video footage may be taken during sessions for the purposes of:

  • Coaching and performance review

  • Internal documentation

  • Promotional material including websites, social media, or printed media
