2023 Summer League Sub Request Form
Thank you for showing interest in subbing. Please fill out the questions to your best ability. We will let you know by Wednesday if you will be needed.

Game day: Monday(open) and Thursday(mixed)
  • Game 1: 7:00 - 8:20 pm
  • Game 2: 8:30 - 9:50 pm
  • Location: Games are at Ford Park in Allen

Cost:
  • The sub fee is $5 and must be paid before you arrive to sub. If we don't receive payment before you play, this may disqualify you from future subbing opportunities.

Payment:
  • Pay the $5 fee to DUA Board PayPal account: duaboard@gmail.com or Venmo: DUA2021. Please do not pay until you have heard that you are able to sub from the league coordinator.

Requirements:
  • Anyone who has not played in a DUA league gets the fee for their first week of subbing waived.
  • All subs will only be able to sub for a max of 3 weeks for winter league.
  • If you want to play for the following weekend, your form needs to be completed by Thursday at 5pm. Your form may not be seen if submitted afterwards.
