This form is to be filled out by the parents of any child that would like to be considered to play on a team of selected players that will play in the Feeder in a future FEEDER basketball season connected to the Edmonds School District area schools (EWHS, Lynnwood HS, Meadowdale HS, MTHS).

All responses will be shared with the ESD Feeder Basketball program coaches, administrators and volunteer staff.
