Thank you for your interest in receiving a 2019 MMLC Funding Award from Expect Miracles Foundation’s Miracle Maker Leadership Council. Please read the following instructions and eligibility requirements carefully before completing your Letter of Intent application.
The information is saved only after all required fields are completed and you click the SUBMIT button on page 7. Upon submitting, you will receive a response email with a link where you can edit your application. To edit the form, first complete the form in its entirety, wait for the confirmation email, and click the grey "Edit Response" button at the top of the email confirmation. You can feel free to make edits until January 28, 2019 at 5PM EST, at this time, the link will be closed and all applications will be considered final.
APPLICATION PROCESS / INSTRUCTIONS:• Review the 2019 MMLC Funding Award Guidelines to determine if your organization is eligible (below)• Submit a Letter of Intent using this google form • Organizations submitting a Letter of Intent application will be notified by February 8, 2019 whether or not they have been invited to apply for a 2019 MMLC Funding Award. Invited organizations will receive the full application.• Once you reach the last page of the application, you can submit. You will receive a response email with a grey "Edit Response" button where you can edit your application. You can feel free to make edits until January 28, 2019 at 5PM EST, at this time, the link will be closed and all applications will be considered final.
CONSIDERATIONS FOR MMLC FUNDING AWARDS:
OBJECTIVE:The MMLC aims to identify and aid innovative organizations and programs that have the passion and potential to make a difference, but lack the resources. The MMLC Program is NOT designed to assist in funding large institutional/national/society entities, but rather local, grassroots, community organizations that are in a period of growth. The MMLC supports organizations where the funding award has the ability to materially impact an organization or program and to further the MMLC’s mission. The maximum Funding Award per year for an organization is $10,000. The MMLC typically funds between 5 - 8 organizations per year. ELIGIBILITY REQUIREMENTS: • Must have current 501(c)(3) status as defined by the IRS • Organizational annual revenue of under $1 million
DO NOT SUPPORT THE FOLLOWING: • Large Institutional/National/Society entities • Private pursuits, individual or team fundraising initiatives • Political parties, associations and representatives of advocacy groups • Organizations that discriminate by race, creed, gender, sexual orientation, age, religion, or national origin • Religious causes • Advertising, promotion, or sponsorship
FOCUS AREAS OF SUPPORT: The MMLC strives to make a meaningful difference in the lives of cancer patients. Priority areas include: o FINANCIAL ASSISTANCEo INTEGRATIVE THERAPIESo LODGING/TRANSPORTATIONo PEDIATRIC/YOUNG ADULT SERVICES
Applications outside the above areas are considered. However, the MMLC Funding Awards are not intended to support cancer research, as a portion of MMLC Funds already benefits the Robert and Renée Belfer Center for Applied Cancer Science at Dana-Farber Cancer Institute. While the MMLC rarely supports one-time events, print projects, and other general administrative expenses, such expenses could be considered if the organization can demonstrate that such expenses have a positive long-term impact for the organization’s clients/patients. We encourage applicants to review past Funding Awards recipients to better understand the types of programs and projects the MMLC likes to support.
GEOGRAPHIC PARAMETERS:The MMLC almost exclusively supports initiatives demonstrating sustainable benefits to communities where Expect Miracles Foundation has a presence and where MMLC members live. These states include: CA, CO, CT, DC, DE, IL, KS, MA, MD, ME, MN, MO, NC, NH, NJ, NY, OH, PA, RI, TX, VT, WA.
Only in rare circumstances will the MMLC choose to fund a program outside these geographic parameters.
