The information on this form will remain confidential and will only be shared with staff working on this project. One-to-One sessions will run for 12 weeks at a cost of £12.50 per session (they normally costs £25-30 per session). In addition you will also receive 4 hours free support to access community based activity.
Criteria for one-to-one subsidised sessions are:
Have a learning disability, autism or mental health issueBe over 16Have a BMI outside of the normal healthy range, as identified by NHS England (normal range = 18.5 - 25)Live in the Ribble Valley
A body mass index (BMI) above the healthy weight range, or too much fat around your waist, can increase your risk of serious health problems like heart disease, type 2 diabetes, stroke and certain cancers. Being underweight could be a sign you're not eating enough or you may be ill.
Group sessions will cost £3.50 per session and will be delivered in community settings or the outdoors. The only restriction to Group Sessions are that you must be 16+
