If you’re sure you’d like to cancel, just complete, and submit this form. This will serve as your 30-day written cancellation notice of your monthly gym agreement. 

Your monthly dues will be canceled 30 days from the submission date of the form below.

*Note that if you have a scheduled payment within this period, the payment will be processed as scheduled. All payments are non-refundable.

If you decide to reactivate your monthly gym dues in the future (and I hope you do) rates in effect at the time of reactivation will be applicable as rates are subject to change.

You will be provided with a confirmation email within the next few days confirming your membership cancellation.
