Please complete this survey so that the arts and culture response community, with support from the Heritage Emergency National Task Force (HENTF), can address the damage to ARTS ORGANIZATIONS caused by the crisis you experienced. The information gathered will help us identify where assistance is sought, provide assistance where needed, understand critical needs, and inform how recovery efforts move forward. This survey should take approximately 10-12 minutes to complete. * = Required response.

PLEASE KNOW THAT YOUR SUBMISSION DOES NOT SERVE AS AN OFFICIAL FEMA ASSISTANCE REQUEST. You should, as soon as you are able, determine if your location has received a Presidential Disaster Declaration and, if so, register for public assistance. This federal government site guides you to FEMA disaster assistance and other resources to help you recover. Declarations are created and updated over time, so check this site for updates.

If you are an INDIVIDUAL ARTIST or PERFORMING GROUP,  please take the ARTIST survey here. If you represent a CULTURAL INSTITUTION, please take the CULTURAL INSTITUTIONS survey here

If you're unsure how to classify your organization's type, refer to the definitions here.

Questions? Please email fema-hentf@fema.dhs.gov

If you are not able to complete this survey online, please email info@ncaper.org to request a paper copy.

The information that you provide in this survey will be used to provide assistance, if you request it. The data will be anonymized and aggregated without any reference to institutions or individuals to provide NCAPER, the National Coalition for Arts Preparedness and Emergency Response and other arts-focused service organizations with a picture of the impact of this crisis on the arts and culture community. If you request assistance, NCAPER, HENTF or one of its members will get back to you regarding your damage and concerns.
