AYSO EXTRA Try-Out Registration Form
AYSO Region 1455 EXTRA Try-Out Registration Form for 2021

Dear Parents/Guardians and Players,

WELCOME to the Ladera Ranch EXTRA Program.  A competitive program in AYSO.

The EXTRA program in Ladera Ranch is one of the premiere competitive programs in youth soccer.  The objective of the AYSO “EXTRA” program is to provide top players within each region a more competitive playing experience than traditionally offered through the recreational primary program alone.  Players participating in this program will compete against EXTRA Play teams from other regions and local area’s during the primary “Extra” season.  Team formations start at 9U-16U for boys and girls.

Acceptance to an EXTRA team is a significant accomplishment and with this privilege comes an obligation on behalf of the player, parent/guardian(s) and our coaches, trainers and team managers.

Our main try-outs occur in December to positions on teams for the following year.  However, we have a few slots open on our teams for Fall 2021.  Specifically, we are looking for players in the following gender and birth years:

Boys 2009       Girls 2010
Boys 2010       Girls 2011
Boys 2012       Girls 2012
Boys 2013

If you are interested in a tryout, please fill out this form ASAP.  Tryouts will be by invitation only in Mid-May 2021.

Thank you,

Neal Karchem

VOLUNTEER INFORMATION
A core component to the success of the EXTRA program is our parent volunteers therefore please let us know if you have been a parent volunteer in the past and if you are willing to be an EXTRA parent volunteer.  All EXTRA teams are required to collect a specific number of referee points throughout the season in order to attend post season tournaments therefore volunteering to be a team referee can be a huge benefit to the EXTRA program and its teams.

PLAYER AND PARENT - General Information
If your child is selected to join one of our EXTRA teams for the year, then the following should be reviewed:

We expect our players and parents to honor their commitments both verbal and written; therefore you should make your decisions thoughtfully and carefully. Once your commitment is made by completing the EXTRA player commitment form, the coach and team members expect your full participation and support for the entire year. Take into account that when you make your commitment, the coach will need to inform other potential players that they did not make the team. A player will not be released from a team during the year unless it is for disciplinary reasons or failure to fulfill their obligations or a season ending injury.

The coaches and trainers will conduct themselves as soccer instructors and mentors.  The coach's and trainer's primary responsibility is for the short and long-term growth and development of your son/daughter as a soccer player and to help our players become world-class players who know how to conduct themselves as positive, responsible and successful members of a team.  You can expect the coaches and trainers to conduct themselves as positive role models and display appropriate behavior. Your coaches and trainers recognize that they are dealing in an important way with young people and we cannot overlook the impact they have on their player's lives.
Field Policies   *
I agree to comply with the posted signs and field requirements of the Owner, City and/or School District, including the presence of pets (dogs).  I also will adhere to any guidelines in place due to COVID restrictions.

AYSO National requires that every player have annual liability/insurance.  If your child did not participate in the Fall 2020 AYSO program, then you will need to sign a liability and COVID waiver prior to attending  try-outs.  If you have selected the box below then an email will be sent to with a copy of the documents  you will need to sign and bring with you to tryout.

Comments
Please make a note if you cannot attend First Looks.  If this is the case, please attend the division closest to your age group for scoring purposes.  (Example: You child should try out for the 2013 team, but instead you try out with the 2012 team for First Looks)
