2017 Mother Seton Honoree Form - Diocesan Home and School Association

The Mother Seton Award was created to honor those individuals whose volunteer service and dedication to home, school and church has been exemplary. This individual should be recognized as an outstanding volunteer and cannot be a paid employee of the school. Strong Catholic values and a selfless dedication to Catholic schools should be manifest in the honoree chosen by your school. Your honoree should have at least three years involvement in your Catholic school community and be an active member of his/her church parish. One volunteer from each school belonging to the Diocesan Home and School Association will be honored at the Home and School Luncheon on Tuesday, April 25, 2017.

A separate Luncheon Reservation Form must also be completed.

The honoree must be approved by your school principal.

Deadline for submission is February 17, 2017.

    Principal's Approval of Honoree

    Please have your principal send an email to Mrs. Christine Francis at cfrancis@csobr.org stating approval of this person as your 2016 Mother Seton Honoree. No honoree submissions will be accepted without this approval.
