Earl Bales Community Day Camp Supplemental Form
As part of our efforts to reduce the number of paper forms and become more technologically efficient, we are beginning to move some aspects of registration to this online supplemental form.
Please note this form does NOT replace the camper application forms found on the "Registration Package" page, but it is one of the required parts of registration.
Please fill this supplemental form out once per camper per summer.

Camp Mailing Lists
During the summer, important information is sent to you via the email address you provide. It is our sole way of communicating important events (e.g. pyjama day tomorrow!) or instructions (e.g. please send your child to camp dressed in a red shirt for Special Day) to you throughout the summer. Once completing the form, you do not need to email any contact information to the camp.
Each camper must have a form entered on their behalf in order for their family to be subscribed to mailing lists. Thank you for your cooperation.

Once again we are pleased to provide PIZZA FRIDAYS at Earl Bales Community Day Camp. We will provide the pizza and ask that you send a drink and whatever else you would like your child to eat with lunch. In order to ensure that we supply the correct amount of each kind of pizza, kindly fill out the form below once for each of your children.
These preferences remain for your child’s entire stay at camp; you do not need to submit a new form for each session. If you do not wish to participate in pizza lunch day, please indicate below and note that you must provide an alternative lunch for your child on Fridays.
