I understand that I am submitting this form as notification that my athlete will not be at practice and a fill in is needed. I acknowledge that I will be charged a missed practice fee for the absence. In addition, I understand that Infinity Allstars does not approve any vacation forms regardless if they do/do not interfere with practice dates, extra practice weekends, choreography, skills camp, competitions, etc.
