VENDOR REGISTRATION: An invoice will be sent to the email address cited herein with the amount due based on options selected after the registration form is received. Please complete the section below for each attendee. Please see the Sponsorship levels below; a progressive number of registrations is included at each level of sponsorship. Birth date - why is this being requested? Paragon Casino Resort will provide discounts on the meeting provisions in exchange for assistance registering conference participants in the Players Club. By providing the name, address and birth date of each person, a VIP Players Club Card will be issued and when 30 points are accumulated, Paragon will credit the card with $10 free play. Pay close attention to your entry to ensure the proper year is submitted
In addition to booth space, vendors interested in sponsorship and underwriting opportunities may contact Gordon Terrell 1-504-595-6039). These sponsorship's assist in funding hospitality functions such as breakfast, coffee service, snacks, social events (bar and/or food), and networking opportunities. Sponsorship allows vendors an added opportunity to display appropriate signage away from their booth and keep their name in front of attendees.
1. If received by date cited, list of sponsors are printed in conference publications provided to each conference participant.
2. Informal tabletop available for sponsor displays.
3. List of LASTO members provided to all sponsors.
4. Sponsors will be formally recognized at lunch on Feb. 20, 2020.
5. Sponsors may bring door prizes for drawing (if interested in doing so).
6. Sponsors will be listed on LASTO website for the upcoming year.
7. Platinum and Platinum Plus Sponsors will have link at LASTO. org to their website.
8. In addition, all Conference sponsors will be listed prominently on the LASTO.org website for 2020, projected on screen between sessions during the 2020 Conference, and included on the sponsor recognition sheet in the attendee registration packet.

A 10' x 10' booth area is included with all levels of sponsorship. This includes an 8' table with linen table cloth, skirt, and 2 chairs. Power outlet upon request.
A 10' x 20' double booth area is available for $250 additional. This includes two 8' tables with linen table cloth, skirt, and 4 chairs. Power outlet upon request.

Sponsorship Level Selection
In addition to your selected level below, each vendor will be assessed a service fee of $35. The fee is required the Paragon to cover costs associated with trade show provisions and for insurance. The following options are available to register your team:

Platinum $1,250 - includes booth plus six (5) registrations
Gold $1,000 - Includes booth plus four (4) registrations
Silver $750 - Includes booth plus three (3) registrations
Bronze $500 - Includes booth plus two (2) registrations
Friends of LASTO $250 - Includes booth plus one (1) registration

Calculating your registration cost for a single payment:
Your cost will be comprised of the $250 per attendee fee, the $35 Service and Insurance Fee, and the optional double booth fee if applicable. An invoice will be sent to the email address cited herein with the amount due based on options selected shortly after the registration form is received.

Paragon Casino Resort Requirements:
Vendors participating in trade shows at the Paragon Casino Resort are required to collect and remit a 9% sales tax to the Tribal Tax Authority on all sales conducted during the conference.
A copy of your responses will be emailed to the address you provided.
