consent to my son or daughter's participation in HOT Team robotics activities and agree to all of the terms and conditions of this Agreement. By signing this Release of Liability, I represent that I have legal authority over and custody of (type your son or daughter's name here)
I have read, understand and agree to enforce the HOT Team Code of Conduct and the HOT Team Participation Requirements. I know that my son/daughter must keep their grades above a 2.8 average and cannot fail any of their classes to fully participate in HOT Team activities. Competition participation is based on academic eligibility, attendance and productivity during the build season. To participate in the state and world championship events, I understand that my son/daughter must participate in at least 20 hours of HOT Team Community Service and at least 90% of the build. I understand that if my son or daughter is not behaving or being productive during the build, he or she will be asked to leave the session early. I understand that should my son/daughter violate the Student Code of Conduct, he/she may be dismissed from the team and/or returned home from any trip, at my expense, at the discretion of the teachers/chaperones involved. I realize that team fees may not be refunded after February 24, 2018. I understand that participation in the FIRST Robotics Program may expose my son/daughter to risks of injury including, without limitation, injury from: building, lifting, and using electrical/mechanical robots and robot components; using tools; and other associated activities. My son/daughter acknowledges and agrees that he/she is primarily responsible for his/her safety.In consideration for my son or daughter being allowed to participate in HOT Team Activities, I assume all risk of participation and hereby release General Motors, Huron Valley Schools and HOT Team representatives from all claims for any injury to my son/daughter (myself) or other damages that may occur as a result of my son/daughter’s participation in the Program and agree not to file any lawsuit or otherwise make any claim against General Motors, Huron Valley Schools or any team representative for any such injury or other damages.In the event of an injury to my son/daughter during HOT Robotics Team activities, I give my permission to the representatives of General Motors or Huron Valley Schools to arrange for all necessary medical treatment for which I shall be financially responsible. This temporary authority will begin on June 24, 2017 and will remain in effect until terminated in writing by the undersigned or June 10, 2018, whichever occurs first.I have read this document and understand that this Consent and Release Agreement includes a waiver of the right to make injury claims that is intended to be legally binding.
