Holiday Gift Assistance Request

Families at Mahtomedi Public Schools who need assistance with holiday gifts may use this form to request support.

To be eligible for this program, you must have a child enrolled in Mahtomedi Public Schools (Early Childhood, K-12 grades and Passages). Non-school age siblings (babies and toddlers) of students enrolled in Mahtomedi Public Schools are also eligible to receive gifts through this program.

Please use this form to indicate each child's needs and interests.

PLEASE NOTE: Gifts your child(ren) receives will be based on the donations we get. We cannot guarantee what or how many gifts your child(ren) will get.

This form will close on November 30, 2024 in order to process requests. All requests will receive a confirmation email the week of December 2nd with details on pick up.

The distribution of gifts will take place December 12-13.
