Booth includes 1- 8 ft. table and 2 chairs: Canopies are available for rent on a limited basis for $25. Please feel free to bring your own. *
All applications are subject to final approval by the Juneteenth Committee and are on a first-come-first served basis. Applicants will receive confirmation by mail or email, if provided. NOTE: It’s your responsibility to have the necessary Salt Lake County and/or Weber County Health Department FOOD HANDLERS PERMIT, TEMPORARY FOOD ESTABLISHMENT PERMITS and/or SALT LAKE CO./OGDEN CITY SPECIAL EVENTS LICENSE, if applicable and comply with their regulations. *If selling items, you are responsible for reporting and paying applicable State Taxes* *
