Request to Apply Student Funds to Trip Payment
Use this form to request funds from your student's Charms account to be applied to the Chorus trip.
You are responsible for any remaining balance due after Charms amount has been applied.
Please submit this form at least five days before the payment is due to allow time for processing.
In order for funds to be transferred, you must have submitted a current Student Fundraising Agreement and paid the $5 program fee.
