- Please complete one form per student.
- For additional students in different grade levels, use a new form. Form links are available from the the School Shop website <
- For additional students in the same grade level, select "submit another form" upon completion.
- If you have more than one student and will be completing more than one form, please fill in the section below.
- Forms are specific to school division and grade level and will provide selection options for all required uniform pieces.
- If you do not require an article of clothing, simply skip to the next item.
- When finished, you may select "send me a copy of my responses" on the last page.
- When finished, close the window.
- When your order is ready for pick-up, you will be contacted at the email address you indicate above.
- It may be helpful to view or print a copy of the Top Marks Sizing Guide and the document for Uniform Daily Requirements before completing the order form. These are available on the School Shop website at <
- Pre-tax prices are listed for each article (tax is calculated at 5% for youth sizing and 15% for adult sizing).
- We are not yet able to offer payment online or through this form.
- You will be contacted at the email address you provide above with your final cost (after-tax) and instructions for payment/pick-up.
