Use this form to request meals for classroom pizza parties, field trips, school breaks and special events.

Please submit requests at least 10 business days before the requested service date.

Submitting this form does not mean the order has been approved. You will receive a confirmation from Community Roots SFSA after the order has been reviewed and accepted.

Changes or cancellations must be submitted through the Meal Order Change or Cancellation Form and must be completed 48 hours in advance or you will be billed for all meals ordered.
