Therapist-Client Agreement
New clients are required to complete this agreement prior to scheduling an appointment. The agreement consists of four sections:
1. Consent to Do Business Electronically
2. Treatment Consent
3. Fees Policy
4. Client Privacy and Records

Boss Wellness, LLC and Client or Parent/Guardian ("the parties") agree that this agreement may be electronically signed. The parties agree that the electronic signatures appearing on this agreement are the same as handwritten signatures for the purposes of validity, enforceability, and admissibility.

A valid email associated with a Google account is required to proceed in order to store protected health information under Google's HIPAA Business Associate Agreement with Boss Wellness, LLC. (HIPAA: Health Insurance Portability and Accountability Act)
