2018 K-8 Cheer Camp Registration Form
Roosevelt Cheer will host a Cheer Camp (K-5) and Stunt Clinic (6-8) on Saturday, January 20th at RHS.
Camp will run from 9am-11am.
Cost is $20 and includes a camp t-shirt, snack, and game admission.
Cheer Campers will perform during half-time of the Varsity Boys Basketball game @ 1:30pm.
Camp Registration & payment are DUE by FRIDAY, JANUARY 5TH.

Checks can be made out to RHS Cheer and dropped off at the high school office. (Please include cheerleader name on check).

By completing this form, I, the parent/guardian of the above named participant hereby acknowledge that my child is in good health and I give my approval for my child to participate in any and all activities during the Roosevelt Cheer Camp. I hereby grant my permission for any and all emergency medical and/or first aid to be administered to my child.
A copy of your responses will be emailed to the address you provided.
