REGISTRATION REFUND/DONATION REQUEST

Please complete this form ONE TIME PER FAMILY regardless of the number of players signed up. Do not complete this form per player.

We request that all families complete this form by May 22, 2020 in order to document how you would like to handle the player registration fees paid for the spring 2020 season. You can request to receive a full or partial refund or you can designate to make a tax deductible donation back to the league in the amount of fees paid.

If this form is not completed by May 22, 2020, we will assume you have chosen to donate the full amount of the registration fees paid back to the league.

Refunds will be processed using the same method of payment (i.e. refunds to credit cards or by check for payments made using cash or check) and will be adjusted for discounts or late fees paid at the time of original registration. A refund will not be processed if an outstanding balance is due on your account.

Refunds will be completed by the end of May and we ask for your patience as we have to process each request individually.
