Mt. Lookout Swim Club - Party Request Form
*NOTE* - Party requests will not be accepted or denied prior to April 20, 2020.
This form serves as a request of a particular date/time for a party held at Mt. Lookout Swim Club. A manager will reply to your request within 3 days to let you know if there is availability for your requested day/time.
Please note: Per the Mt. Lookout Swim Club Handbook, Section V.8 -
- Parties must be submitted for manager approval, a minimum 1 week prior to desired party date
- The sponsoring member shall pay a party fee of $50.00, not including guest fees, and sign a waiver of liability
- Single payment covering party fees and guest fees, along with the liability waiver must be delivered to the Club Manager by the sponsoring member/family on the date of the party
- Any party involving over 30 guests (non-members) must be approved in advance by the Board of Trustees. The approval
of the Board of Trustees will be based on numerous factors, including the proposed date, time, age of
guests, and past party behavior of proposed members and guests.
