Clarendon Water Supply Well Sample Request Form
The Vermont Department of Environmental Conservation (DEC) and Vermont Agency of Transportation (VTrans) have been collecting drinking water samples from homes with a water supply well within an area of interest surrounding the Rutland-Southern Vermont Regional Airport in Clarendon. Currently, DEC and VTrans are looking to sample 20 water supply wells along Middle Road from the intersection with Route 7 at the south end, to just north of Otterview Drive on the north end. The samples will be tested for the presence of per- and polyfluoroalkyl substances (PFAS), such as PFOA and PFOS. The following form is provided to help us know who is located within the area of interest and would like their well sampled. We will contact you in the next several weeks regarding sampling of your well. If you have any issues completing the form please contact Michael Nahmias, site manager at 802-522-4595.
