This USA Hockey Affiliate does not assign officials, but is chartered to register and train USA Hockey Officials within our Affiliate. Each league or organization contracts with a scheduler or assigner to schedule USA Hockey–registered officials for their games.

This report is designed to gather information about officials assigned to officiate a specific game. Please use this form to provide feedback on officials who performed exceptionally well, as well as those who, in your opinion, did not meet expectations.

Officials are charged with the general supervision of the game, including conduct before and after the contests they officiate. In the same manner, officials themselves must be held accountable for their conduct before, during, and after each game.

Your comments should focus on the actions of officials that affect the games you observe, whether in a positive or negative way.

For example: if your team loses by one goal and you feel the official made a poor call, this is often not relevant—unless the official misapplied a playing rule. On the other hand, if you see an official who goes above and beyond in their efforts, or one who demonstrates inappropriate conduct, those are items we need to know about.

We appreciate your help and input so we can continue improving communication and education. Your information will be reviewed by volunteers who donate their time and efforts to support hockey within our Affiliate.

All submitted information will be reviewed. Reports submitted with proper contact information will receive a reply within 10 days. Please note that submitted information may also lead to an investigation by the Affiliate.
