Family Report of Sick Student/Covid-19 Positive Test/Quarantine
The Ohio Department of Health is asking families to report positive cases in children Pre-K through 12th grade to their local school district to assist in contact tracing and to minimize further transmission of the Covid-19. Northridge Local Schools will be reporting all Covid-19 related data - both new positive cases AND "close contact" quarantine cases - to Public Health Dayton Montgomery county on a weekly basis.
WE ARE ASKING FAMILIES TO COMMUNICATE NEW POSITIVE STUDENT CASES IN ONE OF THREE WAYS:
1. Complete and submit this Google Form
2. Email the Covid Reporting Hotline -
3. Call the school clinic - 937.275.7469 EXT. 2148.
What are you reporting today?
My student has tested positive for Covid-19
My student has been placed in quarantine as a "close contact" of someone with Covid-19
How were you notified about positive test/"close contact" quarantine?
Public Health Dayton Montgomery County
Another Local/County Health Department (not Montgomery County)
Medical Provider (for example - Primary Care Doctor, Emergency Room, Urgent Care, etc)
If one was given, please list the date your student may return to school.
Please list the names and grades of any other students in the home who may be affected.
Please list the BEST way to contact your family for follow up - phone number (call or text), email, etc.
