Device Replacement Form
Appointments are set up by date and time only, no unscheduled walk-ins will be permitted. One person will be permitted to pickup/drop off devices during the scheduled Date/Time at our Coatesville Area School district office 3030 C.G. Zinn Road, Thorndale. Please place your broken device in the box marked 'Returns'. Please fill out one form for each device.

If you have a broken device, please bring it with you and drop it off in the Box. The serial# is located on the bottom of the device. The Asset Tag is the white CASD tag with 5-7 numbers on it. (see example below) If you do not have a broken device, fill in 'no device' in space below. Please provide as much info as possible.
Example: CASD White Tag# or Serial# - Also printed beside QR Code on device if the tags are missing.
