Pursuant to Section 25-3.5-1204, C.R.S., an authorized entity that operates a CARES program in Colorado shall report on the progress of the program on or before December 31, on an annual basis, starting the year after the program commenced.
For example, a program that commenced in 2018 would report on or before December 31, 2019, and each year after that.
Each report should reflect services provided throughout the previous calendar year, e.g. Jan. 1, 2019 through Dec. 31, 2019.
If you're reporting on more than one calendar year, submit one report per calendar year of operation.
