Please complete this form if you are willing to be contacted by a current Higher Education and Student Affairs (HESA) Program student and co-create a practicum for Spring 2022, Summer 2022, and/or Fall 2022 terms. You are simply joining an outreach list through completion of this form. You may or may not receive outreach from students and are not committing to a particular practicum experience.

Please complete this form by March 1, 2022 for Summer 2022. Please complete this form by August 1, 2022 for Fall 2022.
